Residential Habilitation and Support Services
Thriving Values staff provide personal support for daily living, self-care, and mobility that can take place in the home or in the community. This is generally not for 24-hour care.
Structured Family Caregiving – Level 1
Personal care services provided to dedicated caregivers in the comfort of a caregivers private home with assistance from Thriving Values.
Participant Assistance Care (PAC)
Thriving Values in-home support services to help individuals be successful while living in their own homes or with natural/adoptive families. This can be 24-hour care, or less.
Wellness Coordination
Thriving Values’ RN or LPN staff create, maintain, and monitor a plan tailored to specific medical, behavioral, or psychiatric needs with the goal being to stabilize health, reduce hospitalizations, and prevent emergency room visits. The RN or LPN train Thriving Values DSP staff to assist with daily health monitoring, medication, and care protocols
Remote Supports
(CIH or FSW) a Thriving Values service that uses technology and off-site, trained professionals to assist individuals to live independently while ensuring their safety and security. It provides a non-invasive alternative to 24/7 in-personstaffing, allowing for privacy and increased autonomy. The setup usually includes motion sensors, door/window sensors, smart cameras, medication dispensers, and fall detection technology
Day Habilitation
This is provided outside the home and helps participants develop the skills necessary for independent living

Thriving Values provides one-on-one staff to support individuals in maintaining competitive integrated employment. This could look like job coaching or managing line-of-sight supervision
